Hospice
Hospice is a program with an extra layer of support for patients and families with life-limiting illness. A team of specially trained health care providers works hand-in-hand with patients, their families, and their own personal doctors to provide individual tailored physical care, emotional support, expert pain and symptom management and spiritual support. In addition to the patient’s own doctor a physician with expertise and certification in hospice and palliative care is available to the patient. The goal is to live each day to the fullest in the environment the patient chooses. Bereavement support is provided for 13 months after the death of a loved one for family and friends. In addition, community education programs are available. Hospice is provided through Home Care agencies.
Litchfield County Home Assistance
Licensed home care registry who can help with a range of tasks. Transportation, meal perp, light housekeeping though family respite, dementia care, hospice care. Hourly care shifts ranging from 3 hours to 24 hours. All aides are independent contractors.

Licensed home care agency who can help with a range of tasks. Transportation, meal perp, light housekeeping though family respite, dementia care, hospice care. Hourly care ranging from 1 hour – 24/7 care. All aides are employed by SVNA Home Assistance and experience ranges from companionship aides through Certified Nurses Aides.
